Depending on your own personal circumstances, you may find the following information useful and beneficial particularly for patients who are aged, infirm or immobilised in some way.

Community Liaison Is Here To Help

Holy Spirit Northside (HSN) Private Hospital provides a Community Liaison service to assist with our clients’ experience of a timely and co-ordinated discharge. Our Community Liaison Nurses have extensive knowledge of the community services that can be arranged to provide assistance with areas of difficulty. The Community Liaison Manager is also the DVA Liaison Officer at HSN.

Planning for your discharge from Holy Spirit Northside begins when you start planning your admission. It is sometimes necessary to make arrangements before your admission to hospital to ensure additional support services are in place. Consideration needs to be given to matters such as any limitations you may have and how you will manage your daily activities on your return home from hospital.
