In your own best interests, please read the following carefully.

  1. The Holy Spirit Northside Admission Form will be given to you by your surgeon.
  2. Please call 1300 884 974 as soon as you can to register your details for admission to hospital.
  3. When phoning, please have the form handy. It contains some important information given by the doctor which the hospital’s call centre administration staff will require.
  4. Before calling please make sure you have the following cards available.
    • Medicare card
    • Pension/concession card
    • Private health fund details
    • Safety net number (if you have one)
    • Department of Veterans’ Affairs (DVA) card (if you have one).

Please note all WorkCover, Workers’ Compensation or Third Party Claims will need PRIOR APPROVAL by the paying authority prior to admission (refer below*).

What You Need To Bring To Hospital

Please leave all your valuables at home and be sure to bring the following with you on the day of admission to avoid delays in processing:

  • X-rays relating to the current procedure
  • Consent Form (if you have been given one)
  • Medicare Card
  • Current Concession/Pension Card
  • Private Health Fund Card
  • DVA Card
  • Credit Card (Diners credit card is NOT accepted)
  • *WorkCover or Third Party Approval letter.

Payment Of Incidentals, Co-payments, Excesses etc

As a condition to providing treatment Holy Spirit Northside Private Hospital will require an imprint of your credit card which will be "swiped" on admission to cover any incidentals or other items not covered by your private health fund. The imprint will be held in a secure and confidential cabinet until your account has been finalised.

On admission, be prepared to pay any co-payments and excesses you may have with your private health fund.

On being discharged, the hospital will debit your credit card for any outstanding amounts which are un-claimable or not paid by you or a private health fund. Some local telephone calls are free but all mobile, STD and long distance calls will require you to purchase a phone card. These are available from admissions for $10 each.

The hospital does not accept personal cheques for amounts exceeding $500 or business cheques in payment of a patient's hospital accounts.
